Reporting Period: July 1 - December 31, 2010
The National Library of Medicine (NLM) is the U.S. Member of the International Health Terminology Standards Development Organisation (IHTSDO) and therefore the SNOMED CT Licensing Authority within the U.S. If you are using SNOMED CT as licensed through the UMLS Metathesaurus (see Appendix 2 of the License Agreement for Use of the UMLS® Metathesaurus®) then you may have to complete the reporting form and submit it to NLM.
Per the Affiliate License terms, all Affiliates with SNOMED CT-related activities in non-Member territories must file a Statement of Account. The current Member countries are: Australia, Canada, Cyprus, Denmark, Estonia, Lithuania, The Netherlands, New Zealand, Singapore, Slovak Republic, Slovenia, Spain, Sweden, the United Kingdom, and the United States. If you are unsure whether your activity requires you to complete a report, please complete the report.
The following question may help you determine if you need to complete the report at this time.
- Question - Did you use SNOMED CT in any product or application outside of the IHTSDO Member countries between July 1, 2010 and December 31, 2010?
- If YES, download and complete the Affiliate License Statement of Account (Word). E-mail the completed form to nichsr@nlm.nih.gov by January 31, 2011. Your reporting requirement is fulfilled when you have submitted the form.
- If NO, you do not need to submit a report. No further action is required during this reporting period.
